    Community Health Needs Assessment
    Sutter Auburn Faith Hospital

    Sutter Auburn Faith Hospital has a long tradition of working to improve the health of the communities we serve through our community benefit activities and programs. These efforts are based on needs identified through a community health needs assessment, conducted once every three years, in collaboration with other health care providers, public health departments and a variety of community organizations.

    In 2016, Sutter Auburn Faith Hospital conducted a community health needs assessment (CHNA) to identify the significant health needs in the communities we serve.

    Implementation Strategy

    The CHNA was used to develop Sutter Auburn Faith Hospital 2016 – 2018 implementation strategy, a plan that demonstrates how our hospital will work to address the significant health needs in our community.

    Soliciting for Public Comments

    As your not-for-profit health care partner, Sutter Auburn Faith Hospital is soliciting public comments to help inform the development of its 2019 Community Health Needs Assessment. You have the opportunity to review our current CHNA and corresponding implementation strategy, and submit comments on either document, by March 1, 2019 to SHCB@sutterhealth.org. All comments received will be considered as part of the community input component in the development of Sutter Auburn Faith Hospital 2019 Community Health Needs Assessment.
